Advance Care Planning (ACP) refers to living wills and/or healthcare power of attorney documents. The last time you or someone you love was admitted to the hospital, you were likely asked if you had any advance directives. This is something they ask just in case something were to happen to you while you are under their care. Have you ever thought about what you would want to happen in this situation if you could not speak for yourself?

Dignified Choices Project
Dignified Choices is a free community service project designed to educate the public about ACP, facilitate easy access to the forms and provide notary public services, as available. There is NO COST and no strings attached! I will be hosting virtual and in person events to discuss ACP to organizations, groups, and individuals in January and February 2026. Please contact me to book a spot ASAP!
Stay tuned for updates to the project as we build it from the ground up. We are based in South Carolina and are currently building our team and gathering data. If you are interested in being a mentor or subject matter expert or have experience in Advance Care Planning, we would LOVE to talk to you.

My name is Angela Seigler
I am a Nurse Practitioner pursuing a Doctor of Nursing Practice (DNP) Degree. Dignified Choices Project is my DNP project. I live in the South Carolina Lowcoutry with my family and one tiny elderly dog. Outside of nursing, I enjoy travel and reading.
